Job Description:
A company is looking for an LPN Case Manager in Disease
Management to facilitate a team approach for quality care
delivery.
Key Responsibilities
Conduct clinical evaluations and assessments to determine
individual healthcare needs
Develop and implement case management plans in collaboration with
members and healthcare professionals
Maintain a member case load for regular outreach, management, and
documentation of interventions
Required Qualifications
Graduation from an accredited Licensed Vocational Nurse program
Current unrestricted LVN/LPN license
Access to dedicated workspace and high-speed internet
connection
Working knowledge of healthcare delivery systems and PC
applications
2+ years of recent clinical experience as an LVN/LPN preferred
