  • Candidates with an AZ home address on their drivers license will not be considered - please include a copy in submission. Banner has a specific cover page! Please vet all possible locations with the travelers and add all experienced departments candidates are willing to ACCEPT/work in. This helps us get candidates presented quicker as orders can open/close quick. The form should be accurate and have all requests noted in the highlights section. Start Date should align with or be very close to the Order Start Date listed. All RTO should be listed on the cover page and must be less than 7 days. RTO will not be approved in the first week of assignment or during holidays. All facilities the traveler is open to working in should be listed on the submission. If interested in 'any' location, please include what states. All shifts the traveler is open to working on should be listed on the submission. All departments the traveler is open to working must be checked on the cover page. All candidates submitted should be preclosed and ready to accept. Notifications are required if the traveler withdraws or is no longer available.

    • 24 months minimum of current PCU experience - REQUIRED
    • Certifications must be American Heart Association OR American Red Cross, active upon submission and INCLUDED IN SUBMISSION - REQUIRED
    • No Nursys Hits - REQUIRED
    • Travel experience - Preferred
